Extra benefits are available through
our Value Rx, Value Rx+, Freedom Rx, and Freedom Rx+ packages. The
Rx packages include a prescription-drug benefit along with your
medical benefits. The Rx+ packages include medical and
prescription-drug coverage, as well dental and vision
coverage. The extra
benefits include:
Prescription Drug Coverage – Value Rx and
Freedom Rx Options
There is no deductible. The Rx packages include generic
and brand name prescription-drug coverage. Copayments for a 30-day
supply are $8 for generic and $38 for brand name drugs. With the Rx package, you
will pay a copayment for each prescription drug until your yearly
total drug costs reaches $2,250. Then, you pay 100% until
your total out-of-pocket cost reaches $3,600 (including
copayments). After your drug costs reach $3,600, you will pay a
small copayment or coinsurance.

Freedom Rx+ Options
There is no deductible. The Rx+ packages include
generic and brand name prescription-drug coverage. Copayments for
a 30-day supply are $8 for generic and $38 for brand name
drugs. With the Rx+
packages, you will pay a copayment for each prescription drug. There is no coverage
gap. After your
drug costs reach $3,600, you will pay a small copayment or
coinsurance.
Vision Benefit - Value Rx+ and Freedom Rx+
Options
Routine Eye Exam and Eye Wear Coverage, including a
routine eye checkup every 12 months for a $15 copayment and
contacts or lenses every twenty-four months with no copayment.
Also, frames are covered every 24 months up to $75.
Dental Benefit - Value Rx+ and Freedom Rx+
Options
Dental
Coverage, including preventive, basic, and major dental services.
A deductible and coinsurance apply for basic and major
services.
