Joining Senior Care Plus is as easy as 1,2,3
1.
Easy to qualify for Senior Care Plus if you:
- Live in Washoe
County , Nevada.
- Are eligible for
Medicare.
- Have both Part A
(Hospital) and Part B (Medical) Medicare coverage.
- Do not have kidney
failure, known as end-stage renal disease (ESRD). If you develop
ESRD while a member of Senior Care Plus, you cannot be
disenrolled.
- No health exams or medical
reviews are necessary to enroll.
2. Review
your coverage choices:
- Value Options
(Health Maintenance Organization)
- Freedom Options
(Preferred Provider Organization)

Complete an enrollment application.

to request our benefit information.
If you are eligible to enroll and we receive
a completed application for enrollment by the end of the month,
your effective date of coverage will generally be the first day of
the next month.
