HealthConnect

HealthConnect is a secure, Web-based tool provided by Hometown Health at no charge. It enables participating healthcare providers to manage their Hometown Health members' eligibility, claims and authorization submissions.

HealthConnect Features

  Claims

  • View and sort claims and eligibility information
  • Select claim information by date of service
  • Filter and look up claims data and view it by patient name, account number, claim number, referral number or check number
  • Find the status of a submitted claim
  • View and print the Explanation of Payments (EOP)

  

Referrals

  • Submit and view authorizations electronically

  

Eligibility

  • Filter and look up eligibility status for all Hometown Health and Senior Care Plus members
  • View a member's benefit documents for coverage guidelines and determine out-of-pocket costs, such as co-payments, deductibles and coinsurance amounts.
