Senior Care Plus Primary Care Associates (PCA) Provider Directory Primary Care Associates is an Independent Practice Association. Hometown Health Plan contracts with this network of Primary Care Physicians. If your primary care physician belongs to this network and needs to request a referral for specialty care or for most hospital procedures, he or she will request the referral through Primary Care Associates, not through Hometown Health Plan.
