As a member of Hometown Health, if you have questions about health insurance claims, authorizations for specialty care or other aspects of your Hometown Health coverage, you can sign up for My Hometown Benefits™. This online service from Hometown Health will give you convenient, secure access to your protected health information at any time.
You can view information about your Hometown Health benefits online from our secure Web site day or night, 365 days a year. My Hometown Benefits™ allows you to view the status of your medical claims for the past six months, as well as pre-authorizations.
Also, with My Hometown Benefits™ you can view your eligibility status, including the Primary Care Physician’s name (for HMO members) and specific benefits of your health plan. You can even submit a change of address and request a change of Primary Care Physician online or search for a specialist or other provider in our network

To begin using My Hometown Benefits™, click on the link in the blue bar located on the left side of this page. You will automatically receive an initial user name and password.
