Using your plan > Changing Your Primary Care Physician
1. If you wish to change your Primary Care Physician, you can simply notify Hometown Health's Customer Services Department on or before the 25th of the month.
2. We will accept your change request by phone, fax, or e-mail, and it will become effective the first of the following month.

4. You will receive a new membership card in the mail approximately 7 days from the date you make your Primary Care Physician change with Hometown Health. Please be sure to destroy your old card so that you will present the correct membership card to your new Primary Care Physician.
5. We suggest that you contact your former Primary Care Physician's office and ask them to transfer your medical records to your new Primary Care Physician.
