Your Primary Care Physician is responsible for:
- Your initial and primary care
- Maintaining the continuity of your care
- Initiating referrals for
you to see a specialist or other healthcare provider according to Hometown
Health's Evidence of Coverage or policy.
Primary Care Physicians can include:
- Family or general
practitioners
- Internists
- Pediatricians

Plan recognizes the importance of women having direct access to OB/GYN care. Although this is not considered primary care within Hometown Health's delivery system, a woman may see her OB/GYN for obstetrical and gynecological needs without a referral from their Primary Care Physician. Some procedures, however, may require a referral; please contact Hometown Health's Customer Services Department for specific information.
Primary Care Physician in an IPA
If you selected a Primary Care Physician listed in the section of your Provider Directory titled "Primary Care Associates Provider Listing," this indicates that your Primary Care Physician is part of an Independent Practice Association (IPA). Your Primary Care Physician has contracted with a specific group of specialty providers for your health care needs. Please refer to the Answers to Your Questions for additional information regarding an independent physician association.
