A specialist receives additional training in his or her particular field, such as cardiology, urology, or obstetrics.
You need an approved
referral from your Primary Care Physician before receiving any specialty care.
The referral will indicate how many visits are approved, the time period
within which you will need to make an appointment, and where the services will
be performed. Hometown Health sends a copy of this referral to you and to
the specialist. Some services require prior authorization from Hometown Health
Plan, including but not limited to inpatient care and surgical procedures. Your
physician will request authorization from Hometown Health Plan when
required.

status of your referral, visit HealthConnect on this Web site (password needed),
call your Primary Care Physician or Hometown Health Plan's Customer Services
Department.
