Hometown Health and the service that you have received.
If you have a concern or complaint regarding Hometown Health, you may
initiate a grievance. Hometown Health has established a grievance procedure
designed to provide a prompt, meaningful, and confidential response to your
issues.
You are encouraged to seek immediate assistance from your physician in
resolving questions or concerns that may arise in connection with services or
treatment received through Hometown Health's contracted providers.
