Exclusions: these are items or services that your health insurance plan does not cover.
- Because your health insurance plan does not cover exclusions, you are responsible for paying for these items.These items can include prescription drugs, services, and treatments.
Evidence of Coverage (EOC): this is a document along with your enrollment form and any other attachments, riders, or other optional coverage selected that explains your coverage, what your insurance plan must do, your rights, and what you must do as a member of that health insurance plan.
