To make the most of your health benefits and to avoid unnecessary charges, you want to ensure you are utilizing providers and facilities that are IN-NETWORK for your specific plan.

Finding a healthcare provider or facility in your network is easy. Your ID Card lists the name of your provider network. Match the network name on your ID card to one of the networks listed below. When you click on a link, you will be redirected to the Online Provider Directory where the selected network will be preloaded.
