Formulary – A formulary is a list of covered preferred brand and generic brand drugs provided by your health insurance plan. The formulary is updated regularly and is subject to change. The formulary is also a good source for you and your doctor to determine the best drug for your condition at the lowest out-of-pocket cost to you.

Family and Medical Leave Act (FMLA) – The Family Medical Leave Act (FMLA) is a federal law that guarantees up to 12 weeks of protected job leave for certain employees when they need to take time off work due to various family or medical reasons. These reasons can include a serious illness or disability, having or adopting a child, or caring for another family member.

Full-Time Employee (FTE) – A full-time employee is any employee who works an average of at least 30 hours a week for more than 120 days in a year.
