Provider Partners

Hometown Health is pleased to partner with Friday Health Plans effective January 1, 2021. Friday Health Plans will be leasing Hometown Health’s PPO network. They will utilize Hometown Health’s PPO contracted rates and Hometown Health will be pricing claims based on those rates. If you are contracted with Hometown Health, you are able to accept and provide care to Friday Health Plan members as an in-network provider. We encourage you to register at Friday Health Plans Provider Portal to view member eligibility and claim information.

Preauthorizations/Referrals

  • If a Specialist will be performing any procedures, they must request authorization by submitting a referral directly to Friday Health Plans

  • Utilization Management review is required and documentation may be requested for:

    • Request for Authorization Form

    • Non-covered Services List

    • Notification Preauthorization List
